However, the comparison of procedural and clinical outcomes between IVUS-guided and OCT-guided PCI for patients with the acute coronary syndrome( ACS) has not been fully evaluated. We aimed to investigate it retrospectively.\nMethods: Consecutive ACS patients who underwent primary PCI within 48 hours from symptom onset were retrospectively enrolled (117 OCT-guided and 47 IVUS-guided PCI). Angiographical characteristics assessed by quantitative coronary angiography, procedural complications, and clinical outcomes were evaluated.\nResults: The patients with IVUS-guided PCI were older, and had a higher proportion of Killip IV. The acute gain was similar and procedural complications of PCI did not differ between the two groups (1% in OCT versus 4% in IVUS, p=0.20). The cardiovascular death within 30 days was lower in the patients with OCT-guided PCI than in those with IVUS-guided PCI (1% versus 11%, p<0.05 by Log-rank test). However, multivariate analysis after adjusting for\nconfounders did not show the difference.\nConclusions: In ACS patients, OCT-guided PCI can be performed as effectively and safely as IVUS-guided PCI in terms of procedural complications and clinical
