| 内容記述 | Background: Safety and efficacy of the irrigation flow technology on contact force (CF)-guided circumferential pulmonary vein isolation (CPVI) remain unknown. We investigated the impact of the irrigation flow by comparing the application time to achieve unexcitability by pacing from the ablation catheter tip. Methods: A 56-hole porous tip (PT) irrigation system (ThermoCool SmartTouch Surround Flow; STSF) were compared with a conventional 6-hole catheter tip (ThermoCool SmartTouch; ST). Forty-two patients with atrial fibrillation( mean age 61±10 years) undergoing CPVI were randomly assigned to either STSF( PT group; n=21) or ST (non-PT group; n=21) catheter ablation. Radiofrequency (RF) application at 30W was performed while pacing at 6 sites along the left CPVI line before the CPVI. The CF was adjusted to maintain 10 g. After the study, PVI was performed with the conventional method. Results: The PT group had a shorter median RF application time from superior to anterior sites than the non-PT group (10 seconds; interquartile range (IQR), 7–17, versus 29 seconds; IQR, 13–51; P<0.01). There was no difference in RF application time from inferior to posterior sites. Except for one patient in the PT group who had esophageal erosion, CPVI was performed without complications. Conclusions: The PT irrigation system is more effective than the 6-hole irrigation tip system in the formation of lesions, especially at the roof-anterior sites. |
