Background: Chemotherapy remains the standard treatment for unresectable pancreatic cancer (UPC), which carries poor prognosis. Although skeletal muscle loss and poor muscle quality have been recognized as prognostic factors in patients with resectable pancreatic cancer, their significance in UPC remains unclear. Methods: We retrospectively analyzed 120 UPC patients who received first-line chemotherapy between 2013 and 2021. The psoas muscle index (PMI) and intramuscular adipose tissue content (IMAC) were measured via computed tomography (CT) before treatment and at the first post-treatment evaluation. Associations between baseline values and early changes and survival outcomes were then evaluated. Results: Low PMI at baseline and first post-treatment CT, but not IMAC and early changes in PMI or IMAC, were significantly associated with shorter overall survival and time to treatment failure. Conclusions: The PMI was an independent prognostic factor in UPC. Baseline muscle mass assessments may aid in risk stratification, and early nutritional or exercise interventions may benefit patients with low PMI. |
