Nutcracker syndrome （NCS） is characterized by that left renal vein （LRV） which becomes stenosis compressed between superior mesenteric artery （SMA） and abdominal aorta. It always induces persistent abdominal pain and hematuria. We present a 23-year-old female with severe persistent abdominal pain. Enhanced computed tomography （CT）showed that LRV was compressed between SMA and abdominal aorta, and it became severe stenosis. Collateral veins to inferior vena cava （IVC） were found from ovarian veins to pelvic veins. Vein angiography showed that there was 3-4 mmHg pressure gradient between LRV and IVC. Left renal vein transposition （LRVT） was performed to release LRV stenosis and persistent abdominal pain with small resection. Left ovarian vein was ligated to decrease perfusion to LRV. Coil thrombosis to collateral veins around LRV was performed postoperatively, and she needed non-steroidal anti-inflammatory drug （NASID） to control abdominal pain at outpatient care. Her abdominal pain was alleviated successfully after pregnancy. |
